- Cause: Pilot Error
- Findings: Crashed on August 3rd, 1973 on a test flight conducted by a Lockheed Test Pilot with a Navy crew member on board. Pilot was demonstrating out-of-trim-elevator handling and lost control of the aircraft and crashed. The Navy crewman may have drifted into the wreckage and died.

Aviation-safety.net source: Aircraft stalled at low altitude and ejection was initiated. Both crew-members got full chutes. The Pilot landed in a ditch and was assisted by a passerby. The Co-pilot passed through the fireball destroying his chute. He did not survive.
Air International July 1974: There was no evidence of aircraft or powerplant malfunction. It appears that the test maneuver was initiated at too low of an altitude for controlled recovery.
